A referral to the SNAP® program can be made by calling the Central Intake Line. The SNAP® programs are a voluntary service. Parents are encouraged to contact the Central Intake Line at (613) 937-7627 and ask for an Intake Worker. Teachers, social workers, police and other service providers may facilitate a referral with permission from the parent or legal guardian.

If you live in Cornwall and your son or daughter is under 12 years of age and is having difficulty at home, school and/or in the community and is:

  • Physically Aggressive
  • Verbally Aggressive or Defiant
  • Fights
  • Steals
  • Vandalizes (damanges property)
  • Sets Fires
  • Trespasses
  • Or has had conflict with authority

The SNAP® Under 12 Outreach Project for boys or the SNAP® Girls Connection can help. To make a referral, please call the SNAP® Central Intake Line at (613) 937-7627. SNAP® services are delivered at: 55 Water Street West, Suite LL50, Cornwall, ON K6J 1A1
